摘要
Combining insulin with a sulfonylurea gives additive glucose lowering and raises the risk of hypoglycemia. Chlorpropamide has a long half-life, so hypoglycemia can be prolonged, especially in older adults and in renal impairment.
用白話說明這項交互作用的意義
Chlorpropamide and Insulin degludec-insulin aspart can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Chlorpropamide belongs to Endocrine; Insulin degludec-insulin aspart belongs to Endocrine.
為什麼會發生
Additive effect: chlorpropamide stimulates insulin secretion while exogenous insulin directly lowers glucose.
處置方式
Reduce the insulin or sulfonylurea dose when combining and monitor blood glucose closely. Consider a shorter-acting agent instead of chlorpropamide, particularly in elderly or renally impaired patients.
