Summary
Amlodipine and nicardipine are both dihydropyridine calcium channel blockers, so combining them duplicates therapy. This increases the risk of hypotension, reflex tachycardia and peripheral edema without added benefit. Nicardipine also inhibits CYP3A4 and may raise amlodipine and atorvastatin levels.
What this interaction means in plain words
Amlodipine-atorvastatin and Nicardipine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-atorvastatin belongs to Cardiovascular; Nicardipine belongs to Cardiovascular.
Why it happens
Therapeutic duplication with additive calcium channel blockade, plus CYP3A4 inhibition by nicardipine.
What to do
Avoid concurrent use of two dihydropyridines and select a single agent. If overlap occurs, such as during transition from IV nicardipine, monitor blood pressure closely.
