Summary

Bumetanide with hydrochlorothiazide produces a strong sequential nephron blockade, causing marked diuresis, hypokalemia, hyponatremia, hypomagnesemia and volume depletion. Combined with valsartan, volume depletion increases the risk of hypotension and renal function decline.

What this interaction means in plain words

Amlodipine-hydrochlorothiazide-valsartan and Bumetanide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: High

Why it happens

Additive diuretic effects at the loop of Henle and distal tubule, with volume depletion increasing dependence on angiotensin II.

What to do

Monitor electrolytes, renal function, weight and blood pressure, especially in the first weeks. Reduce diuretic doses if volume depletion or hypotension develops.
