Summary

Hydrochlorothiazide can cause hypokalemia and hypomagnesemia, which markedly increases the risk of sotalol-induced QT prolongation and torsades de pointes. Additive hypotension and bradycardia may also occur with amlodipine and valsartan.

What this interaction means in plain words

Amlodipine-hydrochlorothiazide-valsartan and Sotalol can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Amlodipine-hydrochlorothiazide-valsartan belongs to Cardiovascular; Sotalol belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Thiazide-induced electrolyte depletion potentiates sotalol QT prolongation, plus additive hemodynamic effects of beta blockade and vasodilation.

What to do

Check and correct potassium and magnesium before and during therapy, and monitor ECG and QTc. Monitor heart rate and blood pressure, and consider an alternative diuretic strategy if electrolytes are unstable.
