Summary
Both carbamazepine and hydrochlorothiazide can cause hyponatremia, and together the risk is increased. Severe hyponatremia may cause confusion, seizures and falls. Carbamazepine, as a CYP3A4 inducer, may also lower amlodipine levels.
What this interaction means in plain words
Amlodipine-olmesartan-hydrochlorothiazide and Carbamazepine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-olmesartan-hydrochlorothiazide belongs to Cardiovascular; Carbamazepine belongs to Central Nervous System.
Why it happens
Additive effects on sodium balance, with thiazide-induced renal sodium loss and carbamazepine-related ADH effects, plus CYP3A4 induction affecting amlodipine.
What to do
Check serum sodium at baseline and periodically, especially in older patients and after dose changes. Consider a different antihypertensive if hyponatremia develops, and monitor blood pressure for reduced amlodipine effect.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
