Summary

Hydrochlorothiazide-induced volume depletion and electrolyte loss, plus ARB effects on glomerular filtration, can increase the nephrotoxic risk of gentamicin. Hypokalemia and hypomagnesemia from the diuretic may also add to aminoglycoside-related toxicity.

What this interaction means in plain words

Amlodipine-olmesartan-hydrochlorothiazide and Gentamicin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amlodipine-olmesartan-hydrochlorothiazide belongs to Cardiovascular; Gentamicin belongs to Anti Bacterial.

Evidence level: Moderate

Why it happens

Volume depletion and reduced renal perfusion from the diuretic and ARB increase aminoglycoside accumulation and nephrotoxicity, with additive electrolyte wasting.

What to do

Monitor serum creatinine, gentamicin levels, potassium and magnesium, and keep the patient hydrated. Consider temporarily holding the diuretic and ARB during acute illness or when renal function declines.
