Summary
Hydrochlorothiazide can worsen glucose tolerance and may partly offset the glucose-lowering effect of liraglutide, whereas olmesartan may slightly improve insulin sensitivity. GLP-1 related weight loss and fluid loss may also add to the blood pressure and volume effects of the antihypertensives. The net effect on glycemia is usually small.
What this interaction means in plain words
Amlodipine-olmesartan-hydrochlorothiazide and Liraglutide have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Amlodipine-olmesartan-hydrochlorothiazide belongs to Cardiovascular; Liraglutide belongs to Endocrine.
Why it happens
Thiazide-induced reduction in insulin sensitivity and secretion opposes incretin effects, while angiotensin receptor blockade may modestly improve insulin sensitivity.
What to do
Monitor blood glucose and HbA1c when starting or changing the thiazide dose, and adjust diabetes therapy if needed. Watch for dehydration if GI side effects from liraglutide occur.
