Summary
Aprepitant is a moderate CYP3A4 inhibitor and can raise exposure to tamsulosin, which is partly cleared by CYP3A4. Higher tamsulosin levels may cause more dizziness, orthostatic hypotension and syncope. Dutasteride is also a CYP3A4 substrate, though the effect is less important clinically.
What this interaction means in plain words
Aprepitant and Dutasteride-tamsulosin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Aprepitant belongs to Gastrointestinal Tract; Dutasteride-tamsulosin belongs to Genitourinary.
Why it happens
Aprepitant inhibits CYP3A4, reducing the metabolism of tamsulosin and dutasteride.
What to do
Monitor for orthostatic hypotension and dizziness, especially during the short aprepitant course. Advise patients to rise slowly and reduce the tamsulosin dose if symptoms occur.
