Summary

Propafenone is metabolised by CYP2D6, CYP3A4 and CYP1A2, so aprepitant may increase its levels, especially in CYP2D6 poor metabolisers. Higher exposure could raise the risk of proarrhythmia, conduction delay and CNS effects.

What this interaction means in plain words

Aprepitant and Propafenone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Aprepitant belongs to Gastrointestinal Tract; Propafenone belongs to Cardiovascular.

Evidence level: Low

Why it happens

Aprepitant inhibits CYP3A4, reducing propafenone clearance.

What to do

Monitor ECG (PR and QRS intervals) and heart rate during aprepitant use, and lower the propafenone dose if conduction abnormalities occur.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
