Summary

Idelalisib strongly inhibits CYP3A, one of the two main pathways that clear aripiprazole. Exposure to aripiprazole may increase considerably, raising the risk of akathisia, sedation, and other adverse effects.

What this interaction means in plain words

Aripiprazole and Idelalisib can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Aripiprazole belongs to Central Nervous System; Idelalisib belongs to Oncology.

Evidence level: Moderate

Why it happens

Strong CYP3A4 inhibition by idelalisib reduces aripiprazole metabolism.

What to do

Reduce the aripiprazole dose to about half of the usual dose while a strong CYP3A4 inhibitor is used, and monitor for adverse effects. Return to the usual dose after idelalisib is stopped.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
