Summary

Both drugs prolong the QT interval, and combined use can produce additive QTc prolongation. This raises the risk of torsades de pointes and other serious ventricular arrhythmias, especially with other risk factors.

What this interaction means in plain words

Bedaquiline and Clofazimine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Bedaquiline belongs to Anti Bacterial; Clofazimine belongs to Anti Bacterial.

Evidence level: Moderate

Why it happens

Additive prolongation of cardiac repolarization (QT) by both agents.

What to do

Obtain baseline and periodic ECGs and correct potassium, magnesium and calcium abnormalities. Stop the drugs if serious ventricular arrhythmia occurs or QTc exceeds 500 ms.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
