Summary
Both oxaliplatin and bedaquiline can prolong the QT interval, so using them together may add to the risk of torsades de pointes. Oxaliplatin-related electrolyte disturbances such as hypokalemia and hypomagnesemia can increase this risk further.
What this interaction means in plain words
Bedaquiline and Oxaliplatin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Bedaquiline belongs to Anti Bacterial; Oxaliplatin belongs to Oncology.
Why it happens
Additive prolongation of cardiac repolarization (QT interval) through pharmacodynamic effects.
What to do
Obtain a baseline ECG and correct potassium, magnesium and calcium before starting. Repeat ECGs during co-treatment and consider an alternative agent if QTc exceeds 500 ms.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
