Summary

Bortezomib is metabolized in part by CYP3A4, CYP2C19 and CYP1A2, and fluvoxamine inhibits CYP2C19 and CYP1A2 strongly and CYP3A4 weakly. Exposure to bortezomib may rise somewhat, raising the risk of neuropathy, cytopenias and gastrointestinal effects.

What this interaction means in plain words

Bortezomib and Fluvoxamine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Bortezomib belongs to Oncology; Fluvoxamine belongs to Central Nervous System.

Evidence level: Low

Why it happens

Fluvoxamine inhibits CYP2C19, CYP1A2 and weakly CYP3A4, reducing bortezomib metabolism.

What to do

Monitor for peripheral neuropathy, thrombocytopenia and gastrointestinal toxicity, and reduce bortezomib dose per labeling if toxicity develops.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
