Summary

Bosentan is a CYP3A4 and CYP2C9 inducer and may lower nevirapine concentrations. Bosentan exposure may also change, and hepatotoxicity risk is a concern with both drugs.

What this interaction means in plain words

Bosentan and Nevirapine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Bosentan belongs to Miscellaneous; Nevirapine belongs to Anti Virals.

Evidence level: Low

Why it happens

Bosentan induces CYP3A4 and CYP2B6, increasing nevirapine clearance, and both drugs are hepatotoxic.

What to do

Monitor HIV viral load and liver enzymes closely, and consider nevirapine concentrations. Consider an alternative antiretroviral if control is lost.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
