Summary
Dronedarone inhibits both CYP3A4 (moderately) and CYP2D6, the two enzymes that clear brexpiprazole. Combined inhibition can substantially increase brexpiprazole exposure and the risk of adverse effects such as sedation, akathisia and orthostatic hypotension.
What this interaction means in plain words
Brexpiprazole and Dronedarone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Brexpiprazole belongs to Central Nervous System; Dronedarone belongs to Cardiovascular.
Why it happens
Dronedarone inhibits CYP3A4 and CYP2D6, reducing brexpiprazole clearance.
What to do
Reduce the brexpiprazole dose to about one quarter of the usual dose per labeling when combined with dual CYP2D6 and CYP3A4 inhibition. Return to the usual dose after dronedarone is stopped.
