Summary

Ribociclib is a moderate to strong CYP3A4 inhibitor, and brexpiprazole is cleared partly by CYP3A4 and CYP2D6. Exposure to brexpiprazole may rise, increasing sedation, orthostasis and other adverse effects. Ribociclib also prolongs the QT interval, which adds a pharmacodynamic concern.

What this interaction means in plain words

Brexpiprazole and Ribociclib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Brexpiprazole belongs to Central Nervous System; Ribociclib belongs to Oncology.

Evidence level: Moderate

Why it happens

Inhibition of CYP3A4-mediated brexpiprazole metabolism by ribociclib, plus additive QT effects.

What to do

Monitor for brexpiprazole adverse effects and consider dose reduction if they develop, noting the label advises halving the dose with strong CYP3A4 inhibitors. Obtain an ECG and check electrolytes in line with ribociclib monitoring.

What to ask your pharmacist
