Summary
Tipranavir, taken with ritonavir, inhibits CYP3A4 and can raise brexpiprazole levels. However, tipranavir also has inducing effects, so the net result is less predictable. Higher exposure raises the risk of sedation, akathisia and orthostasis.
What this interaction means in plain words
Brexpiprazole and Tipranavir can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Brexpiprazole belongs to Central Nervous System; Tipranavir belongs to Anti Virals.
Why it happens
Ritonavir-boosted tipranavir inhibits CYP3A4 metabolism of brexpiprazole, with possible partial offsetting induction.
What to do
Use half the usual brexpiprazole dose with strong CYP3A4 inhibitors, and a quarter if a CYP2D6 inhibitor is also present. Monitor tolerability and response.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
