Summary

Perphenazine blocks dopamine D2 receptors, which opposes the dopamine agonist action of bromocriptine. This can reduce its effect in hyperprolactinemia, acromegaly or Parkinson disease. Parkinsonian symptoms may also worsen.

What this interaction means in plain words

Bromocriptine and Perphenazine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Bromocriptine belongs to Central Nervous System; Perphenazine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Pharmacodynamic antagonism at dopamine D2 receptors.

What to do

Avoid the combination where possible. If an antipsychotic is needed in Parkinson disease, choose a low-D2-affinity agent such as quetiapine or clozapine.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
