Summary
Loop diuretics such as bumetanide can modestly raise blood glucose. Combined with dapagliflozin they also increase the risk of volume depletion, hypotension and renal impairment, which in turn may predispose to metformin accumulation.
What this interaction means in plain words
Bumetanide and Dapagliflozin-metformin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Bumetanide belongs to Cardiovascular; Dapagliflozin-metformin belongs to Endocrine.
Why it happens
Loop diuretics may impair glucose tolerance, and additive diuresis with SGLT2 inhibition causes volume depletion and potential renal function decline.
What to do
Monitor glucose, hydration status and renal function, and consider reducing the diuretic dose when starting dapagliflozin. Hold metformin in cases of acute renal impairment.
What to ask your pharmacist
