Summary
Formoterol, bumetanide and corticosteroids can each lower serum potassium, so together they may cause clinically important hypokalemia. This raises the risk of arrhythmia, particularly in patients with heart disease or on digoxin. The effect is greater with high beta-agonist doses.
What this interaction means in plain words
Bumetanide and Formoterol-mometasone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Bumetanide belongs to Cardiovascular; Formoterol-mometasone belongs to Respiratory.
Why it happens
Additive potassium lowering through beta-2-mediated intracellular shift, renal loss from the loop diuretic and mineralocorticoid activity of corticosteroids.
What to do
Monitor serum potassium and magnesium periodically, especially at initiation or during acute illness, and replace as needed.
