Summary

Adding a loop diuretic to an ACE inhibitor and thiazide increases the risk of volume depletion, hypotension and acute kidney injury, especially at initiation. Combined diuretics also increase loss of potassium, magnesium and sodium.

What this interaction means in plain words

Bumetanide and Fosinopril-hydrochlorothiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Bumetanide belongs to Cardiovascular; Fosinopril-hydrochlorothiazide belongs to Cardiovascular.

Evidence level: High

Why it happens

Additive diuresis and electrolyte loss, with ACE inhibition reducing renal perfusion pressure in volume-depleted patients.

What to do

Monitor blood pressure, renal function and electrolytes (potassium, sodium, magnesium) after starting or changing doses. Lower the diuretic dose if the patient is volume depleted.

What to ask your pharmacist
