Summary
Combining a loop diuretic with a thiazide gives strong sequential nephron blockade, causing marked diuresis, volume depletion, hypokalemia, hyponatremia and hypomagnesemia. Together with losartan, volume depletion can also cause hypotension and acute kidney injury.
What this interaction means in plain words
Bumetanide and Hydrochlorothiazide-losartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Bumetanide belongs to Cardiovascular; Hydrochlorothiazide-losartan belongs to Cardiovascular.
Why it happens
Additive diuretic and electrolyte-wasting effects at different nephron sites, with volume depletion increasing ARB-related renal risk.
What to do
Monitor electrolytes, renal function, weight and blood pressure, especially in the first weeks. Reduce diuretic doses or supplement potassium and magnesium if needed.
