Both salbutamol and the loop diuretic bumetanide lower serum potassium. Combined use, particularly with high-dose or nebulized salbutamol, can cause clinically significant hypokalemia and increase arrhythmia risk.
What this interaction means in plain words
Bumetanide and Ipratropium-salbutamol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Bumetanide belongs to Cardiovascular; Ipratropium-salbutamol belongs to Respiratory.
Why it happens
Additive hypokalemia from beta-2 mediated cellular potassium uptake and renal potassium loss from loop diuretic.
What to do
Monitor serum potassium and magnesium and replace as needed. Take extra care in patients with heart disease, those on digoxin or receiving frequent high-dose salbutamol.
