Bumetanide causes renal potassium loss, and vilanterol can further lower serum potassium by moving it into cells. The combination may increase hypokalemia and ECG changes, especially in patients with cardiac disease or if the beta-agonist dose is exceeded.
What this interaction means in plain words
Bumetanide and Umeclidinium-vilanterol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Why it happens
Additive hypokalemia from loop diuretic potassium wasting and beta2-mediated intracellular potassium shift.
What to do
Monitor serum potassium and magnesium, particularly at initiation and in patients with heart disease, and replace as needed.
