Summary

Bupropion is a strong CYP2D6 inhibitor and raises eliglustat exposure, increasing the risk of QT prolongation and arrhythmia. The risk increases further if a CYP3A inhibitor is also present.

What this interaction means in plain words

Bupropion and Eliglustat can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Bupropion belongs to Central Nervous System; Eliglustat belongs to Miscellaneous.

Evidence level: Moderate

Why it happens

Bupropion and its metabolites inhibit CYP2D6, reducing eliglustat clearance.

What to do

Reduce eliglustat to 84 mg once daily in extensive and intermediate metabolizers and monitor ECG. Avoid if a CYP3A inhibitor is also used.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
