Summary
Bupropion strongly inhibits CYP2D6 and can increase clomipramine concentrations. Both drugs also lower the seizure threshold, so seizure risk and tricyclic toxicity such as arrhythmia and anticholinergic effects are increased.
What this interaction means in plain words
Bupropion-naltrexone and Clomipramine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Bupropion-naltrexone belongs to Miscellaneous; Clomipramine belongs to Central Nervous System.
Why it happens
CYP2D6 inhibition by bupropion reduces clomipramine clearance, with additive seizure-threshold lowering.
What to do
Avoid the combination if possible. If necessary, reduce clomipramine dose, monitor ECG and clomipramine levels, and watch for seizures.
