Summary

Bupropion is a strong CYP2D6 inhibitor and eliglustat is a CYP2D6 substrate, so eliglustat exposure rises. Higher levels may prolong PR, QTc and QRS intervals and cause arrhythmia, particularly when CYP3A inhibitors are also present.

What this interaction means in plain words

Bupropion-naltrexone and Eliglustat can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Bupropion-naltrexone belongs to Miscellaneous; Eliglustat belongs to Miscellaneous.

Evidence level: Moderate

Why it happens

Bupropion and its metabolites inhibit CYP2D6, reducing eliglustat metabolism.

What to do

Avoid the combination where possible; if unavoidable, reduce eliglustat dosing per labeling, obtain an ECG and monitor for arrhythmia. Never combine with additional CYP3A inhibitors.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
