Summary
Ketoconazole inhibits several cytochrome P450 enzymes involved in vitamin D metabolism, which can change circulating calcifediol and reduce its conversion to active calcitriol. The result may be altered vitamin D activity and shifts in parathyroid hormone and calcium control.
What this interaction means in plain words
Calcifediol and Ketoconazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Calcifediol belongs to Musculoskeletal; Ketoconazole belongs to Anti Fungals.
Why it happens
Inhibition of CYP3A4 and other vitamin D-metabolizing enzymes (including CYP24A1 and CYP27B1) alters calcifediol clearance and activation.
What to do
Monitor serum 25-hydroxyvitamin D, intact PTH and calcium when starting or stopping ketoconazole, and adjust the calcifediol dose according to results.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
