Summary
Thiazides reduce urinary calcium excretion, and calcitriol increases calcium absorption, so together they can cause hypercalcemia. The risk is greatest with oral calcitriol, high doses or renal impairment, and is much lower with topical use.
What this interaction means in plain words
Calcitriol and Hydrochlorothiazide-lisinopril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Calcitriol belongs to Musculoskeletal; Hydrochlorothiazide-lisinopril belongs to Cardiovascular.
Why it happens
Thiazides decrease renal calcium excretion while calcitriol enhances intestinal calcium absorption, producing additive increases in serum calcium.
What to do
Monitor serum calcium regularly, particularly after dose changes, and reduce calcitriol or stop the thiazide if hypercalcemia develops.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
