Summary

Thiazide-induced hypokalemia and hypomagnesemia increase sensitivity to digoxin and the risk of arrhythmias. Captopril may modestly raise digoxin levels, and renal impairment from ACE inhibition can further reduce digoxin clearance.

What this interaction means in plain words

Captopril-hydrochlorothiazide and Digoxin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Captopril-hydrochlorothiazide belongs to Cardiovascular; Digoxin belongs to Cardiovascular.

Evidence level: High

Why it happens

Diuretic-induced hypokalemia sensitizes the myocardium to digoxin, with possible reduced renal digoxin clearance from ACE inhibitor.

What to do

Monitor serum potassium, magnesium, renal function and digoxin levels, and keep potassium in the normal range.
