Levoketoconazole is a strong CYP3A4 inhibitor and can raise cariprazine and its active metabolites. This increases the risk of akathisia, extrapyramidal symptoms and sedation.
What this interaction means in plain words
Cariprazine and Levoketoconazole can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Cariprazine belongs to Central Nervous System; Levoketoconazole belongs to Endocrine.
Why it happens
Inhibition of CYP3A4 by levoketoconazole reduces cariprazine metabolism.
What to do
Halve the cariprazine dose when combined with a strong CYP3A4 inhibitor and monitor for adverse effects for several weeks. Levoketoconazole may also prolong QT, so consider ECG monitoring.
