Summary
NSAIDs can reduce the diuretic and antihypertensive effect of thiazides. In volume-depleted or older patients the combination may also increase the risk of acute kidney injury.
What this interaction means in plain words
Celecoxib and Hydrochlorothiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Celecoxib belongs to Musculoskeletal; Hydrochlorothiazide belongs to Cardiovascular.
Why it happens
Celecoxib inhibits renal prostaglandin synthesis, causing sodium and water retention and reduced renal perfusion, which opposes thiazide action.
What to do
Monitor blood pressure, fluid status, serum creatinine and electrolytes, particularly in elderly or dehydrated patients. Ensure adequate hydration and use the lowest effective celecoxib dose.
