Summary
Thiazide diuretics such as chlorothiazide lower serum potassium, and beta-2 agonists can drive potassium into cells. Together hypokalemia may be amplified, and this can provoke ECG changes and arrhythmias, especially with high doses of indacaterol.
What this interaction means in plain words
Chlorothiazide and Indacaterol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Chlorothiazide belongs to Cardiovascular; Indacaterol belongs to Respiratory.
Why it happens
Additive hypokalemia from renal potassium loss and beta-2-mediated intracellular potassium shift.
What to do
Monitor serum potassium periodically, replace potassium or add a potassium-sparing agent when needed, and take extra care in patients with cardiac disease.
