Summary
Both drugs have antimuscarinic activity, so taking them together can add to anticholinergic effects such as dry mouth, constipation, urinary retention, blurred vision, confusion and tachycardia. Older or frail patients are at particular risk of delirium and heat intolerance.
What this interaction means in plain words
Chlorpromazine and Trospium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Chlorpromazine belongs to Central Nervous System; Trospium belongs to Genitourinary.
Why it happens
Additive peripheral and central antimuscarinic effects from trospium and the anticholinergic activity of chlorpromazine.
What to do
Monitor for anticholinergic toxicity, especially constipation, urinary retention and cognitive change, and consider a lower trospium dose or an alternative bladder agent if symptoms occur. Avoid in patients with narrow-angle glaucoma or significant urinary or bowel obstruction.
