Summary
Vandetanib causes marked, dose-related QT prolongation and has a boxed warning for torsades de pointes and sudden death. Adding chlorpromazine, which also prolongs QT, further increases this risk.
What this interaction means in plain words
Chlorpromazine and Vandetanib are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Chlorpromazine belongs to Central Nervous System; Vandetanib belongs to Oncology.
Why it happens
Additive blockade of cardiac potassium channels leading to QT prolongation.
What to do
Avoid concurrent use and choose a non-QT-prolonging alternative to chlorpromazine. If it cannot be avoided, perform frequent ECG monitoring and keep potassium, magnesium and calcium normal.
