Summary

Fenofibric acid may inhibit the bile salt export pump, reducing the hepatic efflux of conjugated bile acids. Combined with exogenous cholic acid, bile salts may accumulate in hepatocytes and raise the risk of cholestatic liver injury.

What this interaction means in plain words

Cholic acid and Fenofibric acid can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Cholic acid belongs to Gastrointestinal Tract; Fenofibric acid belongs to Cardiovascular.

Evidence level: Low

Why it happens

Inhibition of the hepatic bile salt export pump (BSEP) by fenofibric acid reduces bile acid clearance from hepatocytes.

What to do

Monitor serum transaminases and bilirubin periodically during combined use. Consider an alternative lipid-lowering agent if liver tests worsen.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
