Summary

Darunavir, usually boosted with ritonavir or cobicistat, and itraconazole are both CYP3A4 substrates and inhibitors, so each can raise the other's concentrations. Higher itraconazole exposure increases the risk of its adverse effects, including negative inotropy, edema and hepatotoxicity.

What this interaction means in plain words

Darunavir and Itraconazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Darunavir belongs to Anti Virals; Itraconazole belongs to Anti Fungals.

Evidence level: Moderate

Why it happens

Mutual inhibition of CYP3A4-mediated metabolism, with additional strong inhibition from the ritonavir or cobicistat booster.

What to do

Limit itraconazole to no more than 200 mg/day with ritonavir-boosted darunavir. Monitor for itraconazole toxicity and consider measuring itraconazole levels.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
