Summary
Dasatinib is a weak-to-moderate CYP3A4 inhibitor and may raise tacrolimus exposure, which has a narrow therapeutic range. Both drugs can also prolong the QT interval, so toxicity such as nephrotoxicity, neurotoxicity or arrhythmia is more likely.
What this interaction means in plain words
Dasatinib and Tacrolimus can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Dasatinib belongs to Oncology; Tacrolimus belongs to Miscellaneous.
Why it happens
Dasatinib inhibits CYP3A4-mediated tacrolimus metabolism, and both drugs have additive QT-prolonging effects.
What to do
Monitor tacrolimus trough concentrations and renal function when starting or stopping dasatinib, adjusting the tacrolimus dose as needed. Obtain a baseline ECG and monitor QTc and electrolytes.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
