Summary
Deflazacort's active metabolite is cleared by CYP3A4, so posaconazole raises corticosteroid exposure and risk of Cushingoid effects and immunosuppression. Both agents can also lower potassium.
What this interaction means in plain words
Deflazacort and Posaconazole can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Deflazacort belongs to Miscellaneous; Posaconazole belongs to Anti Fungals.
Why it happens
Posaconazole inhibits CYP3A4 metabolism of deflazacort's active metabolite; both drugs can cause hypokalemia.
What to do
Reduce the deflazacort dose to about one-third of the usual dose as per labeling while on posaconazole. Monitor potassium and for corticosteroid adverse effects.
