Summary
Tricyclic antidepressants can potentiate the cardiovascular effects of beta-2 agonists and themselves prolong the QT interval. Concurrent use may increase the risk of palpitations, tachycardia and ventricular arrhythmias.
What this interaction means in plain words
Desipramine and Indacaterol can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Desipramine belongs to Central Nervous System; Indacaterol belongs to Respiratory.
Why it happens
Tricyclic-related QT prolongation and enhanced adrenergic cardiovascular response add to the effects of indacaterol.
What to do
Avoid the combination where an alternative exists, otherwise obtain an ECG and monitor heart rate and potassium. Reduce the desipramine dose or switch agents if QTc lengthens or arrhythmia symptoms develop.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
