Summary

Systemic corticosteroids such as dexamethasone raise blood glucose through increased hepatic glucose output and insulin resistance. This can offset the glucose-lowering effect of vildagliptin and worsen glycemic control in patients with diabetes.

What this interaction means in plain words

Dexamethasone and Vildagliptin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Dexamethasone belongs to Endocrine; Vildagliptin belongs to Endocrine.

Evidence level: Moderate

Why it happens

Pharmacodynamic antagonism: corticosteroid-induced hyperglycemia opposes the incretin-based glucose lowering of vildagliptin.

What to do

Monitor blood glucose closely during and after dexamethasone courses and adjust the antidiabetic regimen as needed. Be prepared to intensify therapy during treatment and reduce it when the steroid is tapered.

What to ask your pharmacist
