Summary

Fluoroquinolones can cause both hypoglycemia and hyperglycemia in patients receiving antidiabetic therapy. Vildagliptin alone carries a low hypoglycemia risk, but glucose control may become unpredictable during a course of gemifloxacin.

What this interaction means in plain words

Gemifloxacin and Vildagliptin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Gemifloxacin belongs to Anti Bacterial; Vildagliptin belongs to Endocrine.

Evidence level: Low

Why it happens

Fluoroquinolones alter pancreatic beta-cell insulin release, producing dysglycemia that adds to the glucose-lowering effect of the DPP-4 inhibitor.

What to do

Monitor blood glucose more frequently while on gemifloxacin and advise the patient on symptoms of low and high blood sugar. Stop the antibiotic and review therapy if significant dysglycemia occurs.
