Summary
Fluoroquinolones such as ciprofloxacin can cause both hypoglycemia and hyperglycemia, particularly in patients on antidiabetic therapy. Vildagliptin alone carries a low hypoglycemia risk, but glucose control may become unpredictable when it is combined with ciprofloxacin.
What this interaction means in plain words
Ciprofloxacin and Vildagliptin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Ciprofloxacin belongs to Anti Bacterial; Vildagliptin belongs to Endocrine.
Why it happens
Fluoroquinolones disturb pancreatic beta-cell insulin release and glucose homeostasis, producing additive or opposing pharmacodynamic effects on glycemic control.
What to do
Monitor blood glucose closely when ciprofloxacin is started or stopped, and counsel the patient on symptoms of hypoglycemia and hyperglycemia. Stop the quinolone and choose another antibiotic if significant dysglycemia occurs.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
