Summary

Thiazide diuretics can raise blood glucose and may partly offset the glucose-lowering effect of vildagliptin. Conversely, ACE inhibitors may enhance insulin sensitivity and increase risk of hypoglycemia slightly. Also, ACE inhibitors are associated with a higher risk of angioedema when combined with DPP-4 inhibitors.

What this interaction means in plain words

Hydrochlorothiazide-lisinopril and Vildagliptin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Hydrochlorothiazide-lisinopril belongs to Cardiovascular; Vildagliptin belongs to Endocrine.

Evidence level: Low

Why it happens

Thiazide-induced hyperglycemia opposes antidiabetic action, and DPP-4 inhibition reduces breakdown of bradykinin and substance P, which may add to ACE inhibitor angioedema risk.

What to do

Monitor blood glucose and HbA1c after starting or changing the thiazide dose. Counsel the patient to report lip, tongue or facial swelling promptly.

What to ask your pharmacist
