Summary
Fedratinib is cleared partly by CYP3A4 and CYP2C19. Dexlansoprazole is a substrate of these enzymes and only a weak inhibitor, so any rise in fedratinib exposure is theoretical and has not been studied. Higher fedratinib levels could increase toxicity such as gastrointestinal effects, myelosuppression and Wernicke encephalopathy.
What this interaction means in plain words
Dexlansoprazole and Fedratinib have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Dexlansoprazole belongs to Gastrointestinal Tract; Fedratinib belongs to Oncology.
Why it happens
Possible weak inhibition of CYP2C19/CYP3A4 by dexlansoprazole reducing fedratinib clearance; not established clinically.
What to do
No dose change is generally required. Monitor for fedratinib adverse effects and thiamine status as per routine fedratinib care.
What to ask your pharmacist
