dexlansoprazole + ondansetron

For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Moderate · Evidence level: Moderate

Chronic PPI therapy can cause hypomagnesemia, which increases ondansetron-associated QT prolongation/TdP risk, especially with other risk factors (low K/Mg, structural heart disease, high IV doses).

Mechanism of action

PPI-induced hypomagnesemia predisposes to ventricular arrhythmias; ondansetron blocks cardiac IKr (hERG) causing QT prolongation; electrolyte disturbances potentiate this effect.

Clinical management

If ondansetron is needed during chronic PPI use, check/correct Mg and K first; use lowest effective ondansetron dose (avoid high IV doses); obtain ECG in high-risk patients; consider alternative antiemetic if Mg cannot be corrected.
