Summary

Posaconazole inhibits the CYP3A4 pathway that contributes to diazepam clearance, which can raise diazepam exposure and prolong its effects. Excessive sedation, psychomotor impairment and respiratory depression may result, particularly in older or frail patients.

What this interaction means in plain words

Diazepam and Posaconazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Diazepam belongs to Musculoskeletal; Posaconazole belongs to Anti Fungals.

Evidence level: Moderate

Why it happens

Posaconazole is a strong CYP3A4 inhibitor that reduces the oxidative metabolism of diazepam.

What to do

Monitor for oversedation and consider a lower diazepam dose during posaconazole therapy. Reduce the dose again or choose a benzodiazepine cleared by glucuronidation such as lorazepam if sedation is excessive.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
