Summary
Thiazide-induced hypokalemia and hypomagnesemia sensitize the myocardium to digoxin and can precipitate arrhythmias and digoxin toxicity. ACE inhibitors may also modestly raise digoxin levels, and renal function changes can further alter exposure.
What this interaction means in plain words
Digoxin and Fosinopril-hydrochlorothiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Digoxin belongs to Cardiovascular; Fosinopril-hydrochlorothiazide belongs to Cardiovascular.
Why it happens
Diuretic-induced potassium and magnesium depletion increases digoxin sensitivity, with possible small reduction in digoxin renal clearance from ACE inhibition.
What to do
Monitor serum potassium, magnesium and renal function regularly and keep potassium in the normal range. Check digoxin level and ECG if toxicity symptoms appear or therapy changes.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
