Summary
Leniolisib inhibits OATP1B1 and OATP1B3 transporters, and this may raise systemic exposure to substrates of these transporters. Digoxin has a narrow therapeutic index, so any rise in exposure could increase the risk of digoxin toxicity such as arrhythmia, nausea and visual disturbance.
What this interaction means in plain words
Digoxin and Leniolisib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Digoxin belongs to Cardiovascular; Leniolisib belongs to Miscellaneous.
Why it happens
Inhibition of hepatic OATP1B1/1B3 uptake transport by leniolisib may reduce digoxin clearance, although digoxin is only a minor OATP substrate.
What to do
Monitor digoxin serum concentrations and heart rate/ECG when starting or stopping leniolisib. Reduce the digoxin dose if concentrations rise or signs of toxicity appear.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
