Summary
Venetoclax inhibits P-glycoprotein and can increase exposure to P-gp substrates such as digoxin, especially in the gut. This may lead to digoxin toxicity given its narrow therapeutic index.
What this interaction means in plain words
Digoxin and Venetoclax can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Digoxin belongs to Cardiovascular; Venetoclax belongs to Miscellaneous.
Why it happens
P-glycoprotein inhibition by venetoclax increases intestinal absorption and reduces efflux of digoxin.
What to do
If coadministration is required, give digoxin at least 6 hours before venetoclax and monitor serum digoxin levels and ECG. Reduce the digoxin dose if concentrations rise.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
